Applying a Letter of Guarantee before hospitalization:

    2. Policy No./Member ID
    3. Phone No.
    4. NRC/ DOB
    5. Email
    6. Hospital/ Clinic
    7. Appointment/ Admission Date
    8. Diagnosis/ Brief Medical no.
    9. Treating Doctor Name

Member will receive forms from cashless@aia.com.mm
1. Member Declaration Form
2. Pre-Admission Form
Member to send the forms to cashless@aia.com.mm:
1. Member Declaration Form (signed by member)
2. Pre-Admission Form (signed by attending physician*)
Member will receive a Letter of Guarantee (E-LOG) via email (if the request is eligible and approved)
Member to proceed for admission

LOG application at hospital:

Member request for cashless service at hospital sharing E-Member ID & NRC
(E-Member ID can be found on Myan X app)
Hospital staff will collect the required information* as below to complete & send the Admission form to cashless@aia.com.mm

    2. Policy No./Member ID
    3. Phone No.
    4. NRC/ DOB
    5. Email
    6. Hospital/ Clinic
    7. Appointment/ Admission Date
    8. Diagnosis/ Brief Medical no.
    9. Treating Doctor Name

Once the admission form is submitted by hospital & it is approved, member need to complete the Member Declaration form & signed
Once all the completed documents are received, Letter of Guarantee (E-LOG) will be issued to member & hospital
